New Clients

If you want to be a new patient, we would love to hear from you. We are located in Lemont (State College), PA.

 

Accepted as a new client?  

Step One: log in

Use our patient portal and the login you were given. Allow 15 minutes to schedule your appointment, update your contact information and do the biographical history. Save your login information in order to communicate with your therapist privately, schedule appointments and make payments in the future.

Didn’t get your login or having problems with it? No worries. Arrive about 10-15 minutes early to complete them on paper in the waiting room. Remember to bring your insurance card, medication list and see directions about where to turn for our office.


Step Two: There are a few additional forms

Step Three (insomnia treatment only): Complete a sleep diary

Click HERE for the sleep diary. In order to make the data useful and get the most accurate sleep prescription possible, please complete the diary for the 2 weeks if possible.

Insurance

We are in-network with Highmark Blue Shield, Highmark Freedom Blue/Highmark Senior and Medicare of PA. We also work with out-of-network benefits and accept private pay.

We strongly encourage you to check your own benefits; we have a free form to guide you during that call to gather the important information.

Fee Schedule

  • $185, Initial Consultation (CPT 90791).  This is an extensive evaluation of your symptoms and history which lasts approximately 1 hour.

  • $142, Individual Psychotherapy, full session, CPT 90837. Length of about an hour.

  • $125, Individual psychotherapy, 3/4 session, CPT 90834. Length of 40 minutes.

  • $71, Individual Psychotherapy, 1/2 session, CPT 90832. Length of 20-30 minutes.

We accept the following forms of payment:

  • Cash in exact amounts. We are not able to make change.

Many of our clients use out-of-network benefits or pay out of pocket. This reflects clients’ decision that our services meet their needs in ways that available in-network providers may not. We do this as an intentional decision on how we choose to spend our time; we put extra time into our clients at a price point that makes high-quality care sustainable, rather than seeing high numbers of in-network clients with more insurance restrictions than we can handle well.We expect payment at the time of service unless other arrangements have been made prior to the session. Your prompt payment helps us to run smoothly and continue to provide a high-quality of service.
